SMS TERMS AND CONDITIONS

These SMS Terms and Conditions ("Terms") govern the use of text messaging services provided by Realization Center, Inc. ("Realization Center," "RCI," "we," "our," or "us"). By providing your mobile telephone number and consenting to receive text messages from Realization Center, you agree to be bound by these Terms.
- Program Description
Realization Center may use SMS (text messaging) communications to contact current, former, and prospective patients regarding healthcare-related matters, including but not limited to:
- Appointment reminders
- Appointment confirmations
- Appointment scheduling and rescheduling notifications
- Intake and admissions communications
- Treatment-related reminders
- Requests for information or documentation
- Patient satisfaction and quality improvement surveys
- Program updates and service notifications
- Office closures, delays, or operational disruptions
- Other healthcare-related communications associated with services provided by Realization Center
SMS messages are intended solely to facilitate communication regarding services provided by Realization Center.
- Consent to Receive Text Messages
By providing your mobile telephone number and opting into SMS communications, you expressly authorize Realization Center and its authorized service providers to send text messages to the mobile number provided.
Consent to receive text messages is not a condition of receiving treatment, services, or benefits from Realization Center. Patients may opt out of SMS communications at any time as described below.
- Message Frequency
Message frequency will vary based upon your interactions with Realization Center, appointment schedule, treatment participation, and communication preferences.
Recipients may receive recurring messages depending upon the nature and frequency of services being provided.
- Message and Data Rates
Message and data rates may apply. Charges are determined by your mobile carrier and service plan.
Realization Center is not responsible for any charges, fees, or expenses incurred by recipients as a result of receiving SMS messages.
- Opt-Out Instructions
Recipients may opt out of SMS communications at any time by replying:
STOP
to any text message received from Realization Center.
Upon receipt of a valid STOP request, SMS communications will be discontinued except as otherwise required by law or necessary to process the opt-out request.
Recipients who opt out may continue to receive communications through other methods, including telephone calls, mail, email, patient portal communications, or in-person communications, as appropriate.

- Carrier Disclaimer
Mobile carriers, wireless providers, and telecommunications service providers are not liable for delayed, failed, intercepted, misdirected, incomplete, or undelivered messages.
Delivery of SMS messages is subject to effective transmission by the recipient's wireless carrier and may be affected by factors outside the control of Realization Center.
- Privacy and Confidentiality
Realization Center is committed to protecting the privacy and confidentiality of patient information in accordance with applicable federal and state laws, including, where applicable, the Health Insurance Portability and Accountability Act ("HIPAA"), 42 C.F.R. Part 2, and other applicable privacy laws.
Information collected through the SMS messaging program is governed by the Realization Center Privacy Policy.
Realization Center may utilize electronic health record systems, communication platforms, business associates, vendors, and service providers to facilitate the delivery of healthcare services and authorized communications on its behalf. Such entities are only permitted to access, use, or disclose information as necessary to perform services for Realization Center and in accordance with applicable privacy and security requirements.
No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Information sharing with subcontractors, vendors, business associates, electronic health record providers, and communication service providers is permitted solely to support the delivery of healthcare services and authorized communications.
Text messaging originator opt-in data and consent will not be shared with any third parties, excluding aggregators, carriers, electronic health record platforms, communication service providers, and vendors necessary to facilitate the delivery of text messaging services.
All categories of information sharing described above exclude text messaging originator opt-in data and consent. Such information will not be shared with any third parties except aggregators, carriers, electronic health record platforms, communication service providers, and vendors necessary to deliver SMS communications.

- No Medical Advice or Emergency Communications
SMS messages are provided solely for informational and administrative purposes. Text messages are not intended to provide medical advice, diagnosis, or treatment.
SMS messaging should not be used to communicate urgent medical concerns or emergencies.
If you are experiencing a medical emergency, call 911 immediately or proceed to the nearest emergency room.
- Modification of Terms
Realization Center reserves the right to amend, modify, or update these Terms at any time. Any revisions will become effective upon posting to the Realization Center website.
Continued participation in the SMS program following the posting of revised Terms constitutes acceptance of such revisions.
- No Guarantee of Availability
Realization Center does not guarantee that SMS services will be available at all times or free from interruption, delay, or error.
SMS communications are dependent upon third-party telecommunications providers and systems that are outside of Realization Center's control.
